Nurseplus Healthcare Services

Nurseplus provides specialised and personalised community based nurse led care for people with complex medical needs, often following serious injury or progressive neurological conditions. By delivering such personalised, comprehensive care in the home, the Service User is able to live at home, maintain independence and achieve a better quality of life within the community.

Nurseplus works collaboratively with the NHS and social care professionals as well as legal and financial institutions to create care services that allow individuals to leave hospitals and restart their lives. Nurseplus care services are tailor made to meet the individual medical and social care needs.

Individuals who may benefit from Nurseplus Support?

Nurseplus provides three core services:

Hospital to Home

The transfer of patients from a hospital environment to care at home that is managed by a specialist Nurse led team. Nurseplus manages this process through a step-by-step process to provide a seamless transition from hospital to a safe and secure home environment.

Home Healthcare

We build close, long-term partnerships with Service Users who have complex medical and/or physical needs and require up to 24 hours Care per day 7 days per week. Nurseplus focuses on the care of patients, managing risks and working towards a greater degree of independence, whilst delivering high standards and continuity of care.

Away from Home Care

We develop tailored services to allow Service Users to attend activities, events and educational facilities thereby allowing them to engage with the wider community and promote greater independence. Recent work has include supporting adults with support needs to attend University.

All our services are provided by a highly trained and qualified multidisciplinary team who take a holistic approach to care provision. We firstly meet the Service User, the family or if appropriate a case manager or other significant interested parties – to fully understand the Service User requirements.

Spinal injury and paralysis
Quadraplegy/paraplegia
Acquired brain injury
Celebral Palsy
Dementia
Degenerative illnesses including Motor Neurone Disease, Huntingdons and Multiple Sclerosis
Short, medium and long term post operation care
IV Therapy

We recognise that we have a huge responsibility to our Service Users, to work with them to plan and tailor their individual care needs and to then ensure that we deliver consistently high quality care at a reasonable cost. We also strive to ensure that the care we provide allows our Service Users to live as independently as possible and that in doing so we can enhance their lives for the long term.

What Services are Available?

The Nurseplus team and our partners have the advanced clinical assessment skills and experience to manage a wide range of complex cases.

We can undertake:

Complete management of tracheostomies and PEGs (feeding tubes) including changeover
Ventilation, Respiratory care, Airway and Medicine management
Urethral and supra pubic catheter care
Stoma care
Epilepsy, Diabetes management
Chest physiotherapy
Delivery of therapies such as speech and language and physiotherapy

We can create care packages within a range of and mix of funding regimes, including insurance funded, direct payments, social/government funded and private payments.
